The Frontier Integrated Care Solutions (F.I.C.S.) program provides free information, education, support and care coordination services for patients with chronic illness.
Patients and their loved ones are welcome to contact the F.I.C.S. team at anytime with questions and/or requests for further information. You do not need a doctor's order, a referral or any insurance to receive information and support from the F.I.C.S. care team.

Chronic or terminal illness
Functional decline or unmanaged symptoms
Seeing several different doctors
Recently discharged from the emergency room, hospital, nursing home or home health care
